[grsec] grsec proc bug since 2.4.23 resurfaced

The problem was due to enabling CONFIG_GRKERNSEC_PROC but neither the
user or user/group /proc restrictions.  The documentation was perhaps not
clear enough that one of the options had to be enabled, so I've clarified
it, and fixed the issue in the code that results if the documentation isn't
read.
